Patient needed both gland removal and areola reduction.  This case demonstrates a complex maneuver where Dr. Khalifeh both removes the gland while also removing excess areola in a circumareolar technique.  The complexity of this case lies in the preservation of good blood flow to the nipple, because the surgery is happening both above (skin) and below (gland removal) and the middle layer needs to be protected just right to preserve the blood flow.
